Overview

The purpose of this activity is to engage hospital teams in a collaborative, case-based learning experience focused on improving the safety and quality of postpartum care transitions. Through ECRI-facilitated Safe Table discussions using real-world perinatal case studies (e.g., hypertension, hemorrhage, and/or sepsis-related readmissions), participants will identify system-level opportunities to strengthen discharge processes, promote early postpartum follow-up, enhance patient debriefing, and improve community resource connections. Small-group discussions will encourage interdisciplinary learning and the exchange of implementation strategies aligned with the AIM Postpartum Discharge Transition patient safety bundle, with key lessons synthesized and shared to support ongoing statewide quality improvement efforts.

Objectives
 
Upon completion of this activity, participants should be better able to:

 


  1. Identify common system and workflow factors that contribute to preventable postpartum complications and readmissions.
  2. Analyze case-based maternal safety events to identify opportunities for improving postpartum discharge planning, care transitions, and interdisciplinary communication.
  3. Apply evidence-based strategies to develop or refine standardized workflows for scheduling early postpartum follow-up, providing patient education and debriefing, and connecting patients with appropriate community resources.
  4. Develop at least one implementation strategy that aligns with AIM Postpartum Discharge Transition Patient Safety Bundle recommendations to improve postpartum care.
