Objectives
  Upon completion of this activity, participants should be better able to:

  1. Improve interprofessional communication amongst all providers
  2. Identify known precipitants of harm
  3. Identify strategies to reduce error
  4. Identify high risk scenarios prone to error
  5. Debrief after a near-miss
