Objectives
 
Upon completion of this activity, participants should be better able to:

 


  1. Describe key Social Determinants (SDOH) and their relevance to medication therapy problems (MTPs) and Advance Pharmacy Practice Experience (APPE) documentation requirements.
  2. Identify SDOH factors during encounters and guide students in linking them to MTPs.
  3. Demonstrate how to coach students in documenting SDOH-related interventions using the SBAR (Situation, Background, Assessment, Recommendation) method.
  4. Evaluate APPE student documentation for completeness, accuracy, and appropriate inclusion of SDOH considerations.

This knowledge-based activity prepares pharmacy preceptors to effectively teach and model Social Determinant of Health (SDOH) competencies during Advanced Pharmacy Practice Experience (APPE) rotations by exploring how SDOH influence medication therapy problems (MTPs), how students should document SDOH considerations in the Ernest Mario School of Pharmacy (EMSOP) learning management platform (CORE ELMS), and how the Situation, Background, Assessment, Recommendation (SBAR) method can be used to structure SDOH-related assessments and recommendations.
