LAST DAY TO REGISTER, WED, NOV 13TH / In-Person Registrants Welcome:   The goal of this CE Conference is to assist Pharmacist attendees and participants in gaining the ability to recognize challenges facing healthcare within New Jersey and areas for collaboration among different stakeholders; to understand the importance of healthcare data within population health and the benefits of patient risk stratification, predictive analytics, and reporting of outcomes; to describe best practices within drug transparency, consumer engagement, and value-based models to improve care quality, clinical outcomes, and efficiency all while reducing healthcare costs; to identify trends within diabetes and collaborative solutions among stakeholders to improve outcomes.

Objectives
Transformation of healthcare to new and innovative models requires development of new strategies and processes and identification of nationally recognized best practices.  These new initiatives and programs will serve to ultimately improve health outcomes, manage healthcare costs, and improve the consumer experience.

  1. Recognize challenges facing healthcare within New Jersey and areas for collaboration among different stakeholders.
  2. Understand the importance of healthcare data within population health and benefits of patient risk stratification, predictive analytics, and reporting of outcomes.
  3. Name and describe best practices within drug transparency, consumer engagement, and value-based models to improve care quality, clinical outcomes, and efficiency all while reducing healthcare costs.
  4. Apply nationally recognized best practices in population health management activities.
  5. Recognize trends within diabetes and collaborative solutions among stakeholders to improve outcomes.
  6. Recognize trends within rheumatoid arthritis and collaborative solutions among stakeholders to improve outcomes.

Registration

GENERAL ADMISSION:  $185.00

Before you register, please be aware that if you are seeking CE credits, you will be required to bring your personal smartphone, tablet or laptop in order to complete the mandatory pre-test and post-test for this CE program, as well as the evaluation.  The pre-test and post-test will only be available during the length of the CE Program.  The evaluation portion will be available for two weeks after this CE program, ending December 2, 2019.

If you do not complete the pre-test and post test during this CE program, you will not receive credit.  The evaluation MUST be completed withing the 2-week period (December 2, 2019) or you WILL NOT receive credit.
